Before Questionnaire Filing: You will need the Unique ID printed on the Class Notice that you received via email. If you did not receive a Class Notice or have lost yours, please contact the Claims administrator at (888) 896-8045 for assistance.

During Questionnaire Filing: If you wish to potentially receive money, you must submit a completed Questionnaire to see if you qualify. The deadline for submitting a Questionnaire is February 27, 2026.

After Questionnaire Filing: After submitting your completed Questionnaire online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to them if you have any questions about your Questionnaire.
